"Ed Cunningham For Mayor" is a Campaign Committee established in Surprise, Arizona to support Ed Cunningham as a write-in candidate for mayor.
Campaign Contributions
The committee is legally required to request identifying information from each contributor. The committee is legally required to report the identifying information for any contributor when the total contribution for the election cycle reaches $100 or more. This includes full name, address*, employer, and occupation.
When making any contributions to this campaign you agree that:
1) The contribution is made from your own funds, and not those of another.
2) The contribution is not made from the general treasury funds of a corporation, labor organization or national bank.
3) You are not a foreign national who lacks permanent resident status in the United States.
4) The contribution is not made from the treasury of an entity or person who is a federal contractor (does not apply to personal contributions by subcontractors, employees, partners, shareholders or officers of federal contractors).
5) The contribution is made from a personal credit card, bank account or debit card for which you have legal obligation to pay, and is made neither on a corporate or business entity account nor on the account of another.
6) You are at least 18 years old unless your parent is making the contribution on your behalf and those contributions go against the parents total contribution amount.
7) You have not exceeded your total campaign contribution maximum.

The committee shall clearly ask for identification and inform the contributor that the committee is required by law to seek identification. Notwithstanding section 16-901, paragraph 29, subdivision (a), a person whose residential address is protected from public disclosure pursuant to section 16-153 is not required to disclose the person's residential address and shall instead provide an alternate mailing address. The committee shall report in an amended report any contributor identification obtained after the contribution has been disclosed on a campaign finance report.
